- The form, WhatsApp, phone and email links on the website are only for appointment and general information requests; they are not a remote examination or medical consultation service.
- The form asks only for your full name, your phone number, optionally your email address, how you would like to be contacted and, optionally, the topic you would like information about.
- Please do not write health information on the form or in emails. On WhatsApp, share only as much as is needed for the appointment; the details are discussed at the examination.
- After the form is submitted, you will be contacted as soon as possible; no specific response time is committed to. If you entered your email address, a single email is sent to let you know that your request has been received; no promotional or campaign messages are sent to you.
- If you are filling in the form on behalf of someone else, that person must be aware of it.
